COMMUNITY COORDINATOR (Fixed Term)
This role will work as part of a team who complete InterRAI assessments and explore options of support for people predominantly aged over 65 years, facilitate access to support for people who are considered Palliative, and provide support for triaging referrals.
The successful applicants will have knowledge and/or experience working in the health and disability sector, ability to effectively engage with Maori clients and whanau, excellent communication skills, with IT skills to an intermediate level. A Health qualification with assessment in scope of practice with a current practicing certificate (i.e. OT, PT, RN or SW) and a clean drivers' licence is essential.
We have available a fixed term full time position, working 80 hours per fortnight, Monday to Friday, with the requirement to participate on the after hours and weekend on call roster.
