FOCUS provides assessment and access to support services, funded through Wairarapa District Health Board (DHB) and Ministry of Health (MoH) for people with long term health and disability needs in the Wairarapa DHB region.

FOCUS is seeking expressions of interest for the role of Post Discharge Navigator to join our unique team.

The focus of this role is to identify people presenting or admitted to Wairarapa Hospital who have an identified risk of readmission and provide a proactive approach to reduce risk factors for readmission and post discharge support for their safe and sustainable return home. The position holder will also participate in the on-going service development for the Post Discharge Navigation Service with the project team.

This is a fixed term full time position until 30 October 2021, working 40 hours per week, Monday to Friday

We would welcome interest from people who have:

• A passion for working with people in a community context.
• Knowledge and experience of working across hospital and community settings.
• Excellent communication skills.
• A flair for critical thinking and problem solving
• A current Annual Practising Certificate as a registered Health Professional
• Competency for completing interRAI assessment
• Computer and keyboard skills in Microsoft office up to immediate level.
• A current full NZ driver's license
